Existing Establishments
- Submit completed application and fee.
- All new employees and management hired after 1/01/2005 must attend or have documentation of completionof an approved Food Safety Course within four months of hiring.
New or Extensively Remodeled Establishment
- Submit completed application and fee.
- Submit building plans showing plumbing, equipment, and finish schedules.
- Submit manager/employee list.
- All new employees and management hired after 1/01/2005 must attend or have documentation of completionof an approved Food Safety Course within four months of hiring.
Change of Ownership or Re-Opening of Establishment
- Submit completed application and fee.
- All new employees and management hired after 1/01/2005 must attend or have documentation of completionof an approved Food Safety Course within four months of hiring.
